PAYROLL DEDUCTION FORM March 2024

As an employee of the Foothill-De Anza Community College District, you can make a voluntary monthly deduction from your paycheck to help strengthen our colleges. On a regular basis, even a small but steady contribution can add up!

In-Kind Gift Form

Complete this form if you have a donor who gave equipment or supplies to your division. Please note, that for any equipment valued more than $5,000, it is up to the donor to obtain an independent appraisal to qualify as a tax deduction.

Foothill-De Anza  Memorial Tree Request Form

Friends and family of loved ones who have contributed to the colleges’ history may purchase a bronze leaf for the Memorial Tree located on the Foothill campus, on the the outdoor wall of Appreciation Hall, adjacent to the fountain.
