Innovation Grants

Foothill-De Anza Foundation Innovation Grants are designed to fund projects related to student equity and retention, ultimately increasing student success. Open to all Foothill-De Anza employees, projects may be submitted by  individual staff or faculty member or by small groups, but funded proposals are likely to involve collaboration among departments and/or between both colleges. Deadline to apply is November 3, 2014.

Application 

New Funds Approval Form

Use this form when considering a grant proposal. Before applying for a grant, all proposals must be approved in order to avoid duplicate submissions to grantors.

Payroll Deduction Form

As a employee of the Foothill-De Anza Community College District, you can make a voluntary monthly deduction from your paycheck to help strengthen our colleges. On a regular basis, even a small but steady contribution can really add up!

In Kind Gift Form

Complete this form if you have a donor who gave equipment or supplies to your division. Please note, for any equipment valued more than $5,000, it is up to the donor to obtain an independent appraisal in order to qualify as a tax-deduction.

Foothill & Central Services Memorial Tree Request Form

Friends and family of loved ones who were Foothill or Central Services employees may purchase a bronze leaf for the Memorial Tree located on the Foothill campus.
